Congratulations on moving forward to the Oral Board Interview and Written Essay Exam portion of the hiring process with the Glendale Police Department!  Pending the results of the Interview and Essay, you may be selected to move forward to the background phase.  

To begin this portion of the process, you will need to complete the online AZPOST Personal History Statement (PH) and SHARE with Glendale Police Department, and fill out the Glendale Police Department Applicant Questionnaire listed below.  You must bring your completed Glendale Police Department Applicant Questionnaire to the Oral Board Interview.

Please print out the Glendale Police Department Applicant Questionnaire and complete it in your own handwriting.  You MUST write your AZPOST Form ID number on the first page of the Glendale Police Department Applicant Questionnaire.

Please note, there is a document on page 3 of the Glendale Police Department Applicant Questionnaire titled ‘Authorization for Release of Information’.  This form must be notarized prior to your Oral Board Interview.  This is in addition to the same waiver you upload on the AZPOST website.  Also note, there are Glendale Police Department Clarification Forms (starting on page 28).  Full explanations are required on all background questions that are answered ‘yes’ (or ‘no’ where indicated) on the questionnaire OR if you feel a background question requires additional explanation.  If in doubt, provide an explanation.
