City of Glendale - News Media Only Request for Police Public Records

Please read the following and acknowledge with your initials at the bottom of the online form: Under provisions of ARS 39-121, Public Records Law, it is requested of the City of Glendale to release the requested public records to me. Under penalty of perjury, I hereby declare the public records requested will be used solely for non-commercial purposes.

Please note, the cost for reports is $0.20/page.
Audio recordings and/or photos will be burned to CD at a cost of $20.00/each. 
File Transfer: $10 transfer fee (includes first 5GB); $1 for each additional 5GB   ****File Transfer option is currently only available for credentialed News Media
